Your Policy

For those members living in one state and working in another, your Policy that applies to you is the policy of the state that your company is located in. For example - if a member lives in Brandon, South Dakota but works in Luverne, Minnesota for Company XYZ, then the Minnesota Policy applies to that member.

If you are an employee of Sanford Health, please refer to the Sanford Intranet and/or Benefits Harbor for your Policy and Summary Plan Description (SPD). The Policy's listed below apply to Sanford Health Plan Commercial members only.
