Your current Medicare Advantage plan will continue exactly as it does today through Dec. 31, 2026. However, it won't be offered in 2027, so you'll need to enroll in a new Medicare Advantage plan to ensure you're covered for next year.

Beginning in early October, you'll receive:

  • Your official CMS-required notice that your plan will not be available in 2027.
  • Additional information about next year's Medicare Advantage plans.
  • Details for local meetings where you can review your options.

Sanford Health Plan and Sanford Health Plan of Minnesota have HMO and PPO plans with a Medicare contract. Sanford Health Plan D-SNPs have contracts with State Medicaid programs.
