Contracting and Credentialing


To become a new participating provider/facility with Sanford Health Plan two steps must be completed:

  1. Contracting
  2. Credentialing

A Provider Contracting Specialist will contact you and send the appropriate participation agreement, provider payment methodology, credentialing forms and W-9 form.

Once you return the completed contracting paperwork, we will begin our credentialing process following NCQA standard guidelines. The provider credentialing applications listed below are for our initial credentialing process.

* Our Sanford Central Verification Office participates with the Minnesota Credentialing Collaborative, which allows credentialing applications to be submitted electronically through the site located at


All Sanford Health Plan participating practitioners and facilities must be recredentialed at least once every 36 months.

Sanford Health Plan will send you a recredentialing application via secure email six months prior to your recredentialing date. Return the recredentialing application to Sanford Credentialing Services.
