Align powered by Sanford Health Plan received 4.5 stars from the Centers for Medicare and Medicaid Services (CMS) for the 2025 plan year.

Medicare Advantage (MA) plans with prescription drug coverage, like Align powered by Sanford Health Plan, usually take several years to achieve even a 4-star rating. This is our second year receiving a star rating, and our second year receiving 4.5 stars – even as CMS adjusted its methodology to make achieving or maintaining a higher star rating more difficult.   

Star ratings are based on up to 40 quality and performance measures, and they are designed to help consumers evaluate the MA plan options available to them. Our 4.5-star rating makes us the highest rated plan in our North Dakota service area and puts us among the highest-rated plans in South Dakota and Iowa. Further, it puts us among the highest-rated plans in the nation as the average rating for MA plans with prescription drug coverage is 3.92 stars with only 7 plans receiving 5 stars.
