Sanford Health Plan (SHP) is updating our process for collecting employer group size and employee counts. CMS requires us to have this information for employer groups at the time of policy application and again at renewal to determine the correct group size and market classification.

Here's what you need to know:

• We will collect this information from all groups in August, then annually effective February 2025.
• We will begin reaching out to clients next week to verify employer group size and employee counts.
  • Clients will be asked to complete this form by Aug. 30; if we do not hear back, we will ask for your assistance in collecting the forms.
• We will also collect this information for new groups through the quote questionnaire and on the new business employer application; both forms will be available on the legacy portal next week.
