Security Health Plan’s ratings by plan in the 2025 NCQA Health Insurance Plan Ratings:

• Security Health Plan’s Medicare Advantage HMO-POS plans rated 4 out of 5 stars in Wisconsin.
• Security Health Plan’s Commercial HMO-POS plans rated 4 out of 5 stars in Wisconsin.
• Security Health Plan’s Medicaid HMO plan rated 4 out of 5 stars in Wisconsin.

Sanford Health Plan’s ratings by plan in the 2025 NCQA Health Insurance Plan Ratings:

• Sanford Health Plan’s Medicare PPO plan rated 4 out of 5 stars in Minnesota, North Dakota and South Dakota.
• Sanford Health Plan’s Commercial HMO plan rated 4.5 out of 5 stars in Iowa, North Dakota and South Dakota.

Sanford Health Plan and Security Health Plan consistently earn top ratings every year from NCQA. The organization implemented the rating system in 2005 and rates plans that choose to report measures publicly. Health plans are evaluated on three categories of quality measures. They include:

1. Clinical quality from NCQA’s Healthcare Effectiveness Data and Information Set (HEDIS®).
2. Patient experience using the Consumer Assessment of Healthcare Providers and Systems (CAHPS®).
3. Results from NCQA’s review of a health plan’s health clinical performance and member experience (NCQA Accreditation).
