ID Card Information
Please remember that on the member's effective date, a new Mobile ID card is available via their member portal, MyChart. You can find more information about accessing the member portal with this helpful flyer. If a member is unable to access a Mobile ID card, please reach out to sales@sanfordhealth.org. We have a team on standby to help. 

Premium Billing

As a reminder, your January 2026 premium payments will be drawn from ACH accounts on the following dates:
 
Small Group -  new clients for January 1st, 2026: if the group does not get an invoice the beginning of January, the January and February premiums will be combined.

  • Bill will be generated: January 20th
  • ACH withdrawal: February 5th

Small Group – renewal clients: January premium only

  • Bill will be generated: December 29th
  • ACH withdrawal: January 9th 

Large Group: January Premium only

  • Bill will be generated: December 29th
  • ACH withdrawal: January 9th

 Please review their invoices, and to reach out to your Account support team with any questions.

Out of Area Verification
To ensure the proper processing of all claims, please remind employees who cover college students or dependents with a permanent address outside of the Sanford Health Plan service area to fill out this form to ensure access to Multiplan's nationwide network. As a reminder, employees enrolled in Sanford TRUE plans are not eligible. If you have any questions about our service area, please reach out our team.

834 File Submission Reminders for Large Groups
Submission Window: December 1–5, 2025

Files submitted after this window may result in processing delays and possible member disruption.

Please review event dates, effective dates, and all enrollment data carefully to avoid billing or coverage issues.

Year-End Reminders: 1095 and W2
1095-B Forms
Individuals are no longer penalized on their income tax return if they don’t have medical insurance; and do not need the form to file their 2025 tax return. However, Sanford Health Plan is still required to have 1095-B forms available by January 31, 2026, for all individuals who had health insurance during the 2024 calendar year.

A reminder that Sanford Health Plan does not mail 1095-B forms. Rather, if members want their 1095-B form, they can request a copy by calling our Customer Service team at (800) 752-5863.

1095-C Forms
If you are considered an applicable large employer (those with 50 or more full-time employees/equivalents), you are required to send 1095-C forms to your employees. For the 2025 calendar year, applicable large employers must provide these forms by January 31, 2026.

Sanford Health Plan will provide data to all applicable large employers to complete Part I and Part II (and Part III for self-funded groups) of the form. The data will be sent from your Sales & Retention representative via a secure email, on or before January 26th, 2026.

W-2 reporting requirements
The ACA requires employers to report the cost of coverage (both employer and employee portion) under an employer sponsored group health plan. Employers are required to report this amount on the employee's W-2. If you need the data for this requirement, please contact your Account Manager.
