If you are considered an applicable large employer (those with 50 or more full-time employees/equivalents), you are required to send 1095-C forms to your employees. For the 2024 calendar year, applicable large employers must provide these forms by January 31, 2024.

Sanford Health Plan will provide data to all applicable large employers to complete Part I and Part II (and Part III for self-funded groups) of the form. The data will be sent from your Sales & Retention representative via a secure email, by Jan. 26, 2024.
