Finish Your Year Confident and in Control
The end of the year can be a hectic time for everyone, and busy schedules make it easy for things to slip through the cracks. That’s why fall is the perfect time to review your health checklist and get caught up on important health planning for next year. Are your tests done? Have you scheduled your next appointment? Do you need to refill any medications? Doing this sooner rather than later can help you avoid gaps in your care and start the new year strong.
Download your Year-End Health Checklist to stay organized and make sure nothing gets missed before the new year.

- Talk with your primary care provider about any remaining concerns
- Review your care plan for the year ahead
Personal Year-End Health Reviews
Taking some time to reflect on your current health status before heading into the new year is so important—how have you been feeling? How has your health changed over the last year? Do you have any health goals you want to take on next year?
Consider these key areas during your review:
Physical Health
Assess your energy levels, sleep quality and ability to perform daily activities while noting any changes from previous years.
Mental and Emotional Health
Reflect on your overall mood, stress levels and sense of purpose or fulfillment while noting any long-term periods of high-highs and/or low-lows.
Preventative Care
Review which recommended screenings, vaccinations and check-ups you’ve completed, and which might be due in the coming year.
Chronic Conditions
Evaluate your current approaches to how you manage ongoing health conditions, and how well they’re working for you.
Health Behaviors
Consider your patterns around nutrition, physical activity, stress management, and other lifestyle factors that influence your overall health.
