Diabetes Prevention

Change your weight with a 12-month weight loss program designed to reduce your risk for Type 2 diabetes.

In this evidence-based program, participants meet in a group with a trained coach to focus on strategies to lose weight, increase physical activity and decrease their risk of developing type 2 diabetes. This program is open to all Sanford Health patients aged 18 or older.

24 Education Sessions with a Coach

Meets weekly for first four months for a total of sixteen sessions, then monthly for next eight months

 

Participants are asked to track their food intake, physical activity and weight every week with a goal to lose 5% or more of their body weight

Connect with others who are also working on improving their health and taking action on changes

Find support to work through challenges from your class and individually from your coach

Are You at Risk?

One in three adults has prediabetes and many do not know it. Take the risk test to see if you are at risk for developing pre-diabetes.

To participate in the Diabetes Prevention Program, individuals must have at least one of the following qualifications:

  • A risk test score of 5 or higher
  • A history of Gestational Diabetes while pregnant
  • A blood glucose (blood sugar) testing result of:
    • Fasting blood glucose (range 100-125 mg/dl)
    • 2-Hour Glucose (range 140-199 mg/dl)
    • HbA1c (range 5.7-6.4%) 

If you have been diagnosed by your physician with Type 1 or Type 2 diabetes you do not qualify for this program. Please visit our Exercise is Medicine program webpage for more change options.

Classes are held in two formats: online or in-person

Online Class
The online program meets using software that is accessible using a desktop computer, laptop, smartphone or tablet. A coach facilitates an engaging discussion with participants during the live class. Participants will receive support from their instructor and fellow classmates who are working on similar goals. All attendees will have access to a workbook with educational information related to each session topic. A scale will also be provided for at home use. The cost of this program is $99 for Sanford Health patients. You may use health savings account dollars to pay for this program. Sanford Health Plan members will have the $99 fee waived as a part of their wellness benefits. The program fee will not be collected until the time of full registration and program commitment. Align powered by Sanford Health Plan members and Great Plains Medicare Advantage members cost is $0 as part of their wellness benefits.

In-person Class
The in-person classes are facilitated by a coach. Participants will weigh in at class and be provided with a workbook with educational information related to each session topic. Participants will receive support from their instructor and fellow classmates who are working on similar goals.
