Effective Aug. 18, 2026, Sanford Health Plan paused Missing Code First Diagnosis Code claim edits Z045.13 and Z045.12.  

These edits identify claims missing a required first-listed diagnosis code. Claims processed on or after Aug. 18, 2026, will not be subject to these edits while they are paused.  

This pause will allow Sanford Health Plan additional time to review and further customize the edits to ensure they align with applicable coding expectations and requirements.  

Please note that this is a pause and does not represent a permanent discontinuation of the edits. Additional information will be shared as our review progresses. Providers do not need to take any action at this time.
