Radiation treatment code updates for 2026

Several radiation therapy CPT and HCPCS codes have been revised or retired effective Jan. 1, 2026. The changes are summarized below along with what action, if any, is needed from providers. This applies to all lines of business. 

Revised radiation delivery codes
The following CPT codes remain active but have updated descriptions. Existing authorizations will be honored.

77402 – Now reflects Level 1 radiation delivery (e.g., simple electron or 2D photon treatments), including imaging guidance.
77407 – Now reflects Level 2 radiation delivery (e.g., single-isocenter 3D or IMRT photon treatments), including imaging guidance.
77412 – Now reflects Level 3 radiation delivery (e.g., multiple isocenters or motion-managed photon therapy), including imaging guidance.

Deleted IMRT delivery codes
The following IMRT delivery codes are retired:
77385 (simple IMRT delivery)
77386 (complex IMRT delivery)

The work previously reported with these codes is now included in 77407. Existing authorizations using the retired codes will be mapped to the revised codes.

Deleted CT guidance code
77014 (CT guidance for radiation field placement) is deleted.

This service is now bundled into the daily radiation delivery codes (77402, 77407, 77412). No action is required.

Deleted HCPCS guidance codes
•    G6001–G6017 (radiation treatment delivery G-codes) are deleted.

Providers may resubmit a revised treatment plan for remaining therapy using the updated CPT codes.
