Change Your Weigh: A Centers for Disease Control National Diabetes Prevention Program

Are you at risk for diabetes? You can change your lifestyle and improve your health. In this evidence-based year-long program, participants meet with a trained lifestyle coach to focus on lifestyle-change strategies to help lose weight, increase physical activity and decrease risk of developing type 2 diabetes.


  • $50 deposit
  • Can be earned back through attendance


  • Meets weekly for first four months
  • Meets monthly for next eight months
